How to Use AI to Design Shift-Handoff Templates (Templates Only, No Patient Details)

Published · 3 min read

Handoff is a safety-critical moment, and many units use a structured format. AI can help you design the layout of a blank template or checklist. It must never be fed real patient details, and it must never write the handoff for a real patient. This guide is part of the AI for Nurses hub.

Caution: Never put patient information (protected health information, PHI) into a general AI chatbot: no names, dates of birth or service, MRNs, room numbers, photos, or any detail that could identify a patient. Follow your employer's AI and privacy policy and HIPAA. AI is not clinical decision support: do not use it for diagnosis, dosing or care decisions. See AI for nurses: HIPAA and patient privacy. This is general information, not legal or clinical advice. Handoff content for real patients belongs in your facility's approved record or handoff tool only.

Templates, not reports

The safe use is a blank form: headings, prompts and a logical order. Common structured formats such as SBAR (situation, background, assessment, recommendation) are widely taught. Your unit or facility may require a specific one, so start from that format and ask the AI only to help lay it out or write short prompts for each section.

What to ask for

  • A one-page blank report sheet with the section headings from your unit's required format.
  • Short reminder prompts under each heading (for example, 'lines and drains', 'pending tests', 'family concerns') that you review against your policy.
  • A checklist version for a quick end-of-shift scan.

What to keep out

No real names, room numbers, ages, diagnoses tied to a person, dates, or anything from a chart. Even a 'made-up' example can drift toward a real case, so use obviously fictional placeholders such as [PATIENT], [ROOM], [DIAGNOSIS].

Get it approved

A handoff form affects care. Before using a new layout, run it past your nurse manager, educator or the committee that owns handoff standards, and store it in the place your facility uses for approved forms.

A template-only prompt

Design a blank one-page shift-handoff worksheet for nurses on a [TYPE OF UNIT, e.g. medical-surgical] unit, organized using the headings of [OUR REQUIRED FORMAT, e.g. SBAR].
Under each heading, add 3 to 5 short reminder prompts as questions or blanks. Use only placeholders like [PATIENT], [ROOM], [DIAGNOSIS]. Do not include any real or example patient information.
Keep it printable on one page. Add a line at the bottom: 'Verify against the chart and orders. This worksheet does not replace the record.'

Check it before you use it

  • The template matches the handoff format your facility requires.
  • It contains only blanks and placeholders, with no real or example patient data.
  • A manager or educator approved it before use.
  • It states that the chart and orders are the source of truth.
  • Real handoffs are recorded only in your approved system.

Frequently asked questions

Can I use AI to write my actual shift report?

No. A real report contains patient information and clinical judgment, and it belongs in your facility's approved system, not a general AI tool.

Is a fake example patient okay in the prompt?

Use placeholders instead. Made-up examples can accidentally mirror a real patient, and they add nothing to a blank template.

Which handoff format should I use?

Use the one your unit or facility requires. SBAR is common, and others exist. Ask your educator or manager.

Can AI check my handoff for missing items?

Not with real patient content in a general tool. You can ask AI to suggest generic checklist items for a template, then verify them against your policy.
